*New Members! – Receive a 20% discount on membership fees in your first year (pro-rated fees also available from January 1 2019 to June 30 2019) – *Apply before June 30 2018 and receive membership benefits in the remainder of the current year (ends June 30 2018) in addition to the 18/19 year!
*Renewing Members! Receive a 20% discount on membership fees when renewing annually by early-bird deadline – July 1 2018

Terminating An OFIS Membership – all OFIS memberships MUST be terminated in writing using this form which MUST be returned to our office by September 30 annually to avoid penalty and recurring membership costs

Membership Application Process:
1. Fully complete the correct application form – including payment information with required signatures. Be sure to include copies of requested documents based on eligible membership level. Incomplete applications will be returned and could result in interruption to membership services.
2. Send completed packages (application, PAD form – if applicable – documentation copies & payment) to: OFIS – 101 Holiday Inn Dr., PO Box 29011, Cambridge, ON, N3C 0E6
3. You will receive an email from our office noting the status of your school’s application within 48 hours of receipt in our office.
